Justice & Community Transition Support

Supporting people with disability to build a stable, connected life in the community after leaving a correctional setting — with dignity, respect and the right supports.

A supported pathway into community life

People with disability who are transitioning from correctional settings often face significant barriers to community participation — including housing instability, limited social connections, disrupted support networks and complex health needs. Together in Motion Services provides disability supports that help people build a stable, meaningful life in the community.

We use respectful, person-centred language and practice. We do not make assumptions about a person's history, and we work collaboratively with the participant, their family, justice workers, support coordinators and other professionals to put the right supports in place.

Who we work with

We welcome referrals from professionals supporting people through the justice transition process.

Corrective services and justice workers

We work with justice workers to understand a person's situation and identify disability supports that can assist with community reintegration.

Support coordinators and LACs

We partner with coordinators to implement supports quickly and maintain clear communication throughout the transition.

Allied health professionals

We work alongside psychologists, OTs, social workers and other allied health professionals to ensure our supports align with the person's broader care plan.

Families and support networks

Where the person wishes, we involve family members and trusted support people in planning and ongoing communication.

Community organisations

We collaborate with Aboriginal and Torres Strait Islander organisations, mental health services and other community organisations to provide culturally safe, holistic support.

NDIS participants

People with disability can contact us directly to explore what supports may be available as they transition into the community.

Accommodation support

We can assist with Medium Term Accommodation (MTA) for people who need a stable place to live while longer-term housing is arranged, subject to availability and NDIS funding.

Assistance with Daily Living

Support with personal care, meal preparation, medication prompting and other daily tasks that help a person live safely and independently.

Community participation

Support to build social connections, engage in community activities and develop a sense of belonging in the local community.

Life skills development

Building practical skills in areas like budgeting, cooking, using public transport and managing daily routines.

Transport

Accessible transport to appointments, services and community activities.

Culturally safe support

We are committed to culturally safe practice, including for Aboriginal and Torres Strait Islander participants. We work with community organisations and cultural advisors to ensure our supports are appropriate and respectful.

Person-centred

We start with the person — their goals, their strengths and what matters to them. We do not define people by their history.

Respectful and non-stigmatising

We use respectful language in all our communications and practice. Every person we support is treated with dignity.

Collaborative

We work as part of the person's broader support network — not in isolation. Clear communication with all parties is central to how we operate.

Culturally safe

We are committed to culturally safe practice for all participants, with particular attention to the needs of Aboriginal and Torres Strait Islander people.

Transparent about capacity

We are honest about what we can and cannot provide. If we are unable to assist, we will try to point you toward other options.

How the referral process works

Make a referral or enquiry

Contact us by phone, email or online referral form. We aim to respond within 2 business days.

Initial conversation

We speak with the referrer and, where possible, the person themselves to understand their situation, goals and support needs.

Capacity and planning check

We confirm current capacity and work with the broader team to identify the most appropriate supports. We are transparent if we cannot assist.

Supports in place

Once agreed, we put supports in place and maintain regular communication with the person and their support network.

A note on funding and eligibility

Together in Motion Services does not determine NDIS funding eligibility or guarantee specific outcomes. All supports are subject to the person having relevant NDIS plan funding and Together in Motion Services having current capacity in the relevant location. We encourage early contact so we can work through options together.
