Together in Motion Services Pty Ltd ("Together in Motion Services", "we", "us", "our") is committed to protecting your personal information. This Privacy Policy explains how we collect, use, store and disclose personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).

What information we collect

We may collect personal information including your name, contact details, date of birth, NDIS number, health and disability information, emergency contact details, financial information relevant to NDIS funding, and information about your support needs and goals. We collect this information directly from you, from your authorised representative, or from referrers such as support coordinators, allied health professionals or hospital discharge teams.

How we use your information

We use your personal information to provide and manage disability support services, communicate with you and your authorised representatives, comply with our obligations as a registered NDIS provider, improve the quality and safety of our services, and meet our legal and regulatory obligations. We do not use your information for marketing purposes without your consent.

Disclosure of your information

We may disclose your personal information to the NDIS Quality and Safeguards Commission, the National Disability Insurance Agency (NDIA), your support coordinator or plan manager, allied health professionals involved in your care, emergency services where required for your safety, and other parties with your written consent. We do not sell your personal information to third parties.

Sensitive information

We handle sensitive information, including health and disability information, with particular care. We collect and use sensitive information only where it is necessary to provide your supports, with your consent, or where required or authorised by law.

Storage and security

Your personal information is stored securely in electronic and physical records management systems. We take reasonable steps to protect your information from misuse, interference, loss, unauthorised access, modification or disclosure. We retain records for the periods required by law and our obligations as an NDIS provider.

Access and correction

You have the right to access the personal information we hold about you and to request corrections if it is inaccurate, incomplete or out of date. To make a request, contact our Privacy Officer at [email protected]. We will respond within 30 days.

Complaints

If you have a concern about how we have handled your personal information, please contact our Privacy Officer in the first instance. If you are not satisfied with our response, you may lodge a complaint with the Office of the Australian Information Commissioner (OAIC) at oaic.gov.au or by calling 1300 363 992.
